Investigation Summary
At approximately 7:00 a.m., a crew of four employees started their shift. The crew was scheduled to finish decking the second level. Employee #1 was shaking out a bundle of plywood on the second floor as the crew was preparing to deck the floor. Two pieces of plywood were used to cover a skylight opening but were not nailed or secured to the building. Also, there was no sign with legible letters to warn the affected employees not to remove that plywood as it had been used as a means to guard that opening. Employee #1 had inadvertently picked up a piece of plywood being used to cover the skylight opening and walked into that opening. Employee #1 fell through that skylight opening and landed on the first floor. Employee #1 fractured his leg and was hospitalized.
