Investigation Summary
On November 4, 2004, Employee #1 was unloading scrap cardboard from a passive roller conveyor side track with his back turned to the path of the transfer car, which traveled on a track. The transfer car was operated by an attendant whose view of the Employee #1's working area was blocked by materials stacked on a section of side track next to the operator's console. Employee #1 was struck by a transfer car holding a section of powered roller conveyor. Employee #1 sustained a fracture to the left ankle, requiring surgery. Employee #1 was hospitalized.
