Investigation Summary
On August 17, 2004, Employee #1 was installing blocking between the roof trusses for Universal Construction Company engaging in electrical, concrete and framing activities. He was not equipped with a personal fall arrest system. Employee #1 was standing on two trusses that measured 4 feet from center to center while installing the blocking to the roof trusses when he somehow slipped and fell 13.5 feet to the wooden floor. The surface of the trusses was smooth and dry when he fell to the wooden floor. He was hospitalized for his injuries.
