Investigation Summary

Investigation Nr: 201058583
Event: 07/08/2004
Employee's finger is amputated while rolling gate

On July 8, 2004, an employee was closing the rolling gate across the receiving area with another management coworker. As they were attempting to close the gate, the coworker noticed some dirt build-up at the bottom of the gate obstructing the track. It was determined the employee was the one pulling the rolling gate while the coworker pushed the gate from the other end. It was also determined that the coworker might had taken his eye off the gate for just a second or two and as the result, the employee's left index finger became caught in between the guide wheel and the guide pole. The employee was taken to Arcadia Methodist Hospital soon after the injury where his injured finger was amputated from the first knuckle after the surgery. He was not hospitalized.
