Employee injured from fall

At approximately 5:30 a.m. on February 17, 2004, Employee #1 was inspecting work previously conducted by a paint crew during the night shift. The work was performed on the St. John's Bridge in Portland Oregon. Below the bridge was an enclosed suspended platform scaffold that was about 130 feet above the ground and was accessed through an extension ladder placed between the bridge and scaffold. Employee #1 climbed down the extension ladder, inspected the work, and proceeded to climb back up to the bridge. As he ascended the ladder, he fell backwards, landing on the suspended platform. Employee #1 was hospitalized and treated for a concussion.
