Employee fell off the ladder and landed on other employee

At approximately 1:15 p.m. on December 16, 2003, Employee #1 was spray painting a stairwell. He placed the ladder on a stair tread and against a riser then climbed the ladder to apply the paint. The ladder, which was resting on the under side of the stair above, slipped to the side and sent Employee #1 off the ladder and to the stairs approximately 16 ft below. He landed on Employee #2. Both employees were knocked unconscious and both sustained other injuries including fractures and concussions and were hospitalized.
