Event: 09/21/2005
Employee Is Killed by Falling Lumber

At approximately 2:10 a.m. on September 21, 2005, Employee #1 was transporting lumber bundles via a forklift from the rip shed to the storage area. The length of the lumber bundles was approximately 16-ft, with bundles stacked four to five high, for a total height of approximately 12- to 15-ft. Each bundle was separated by two spacers of 2-inch by 4-inch. The bundles were banded just once at their approximate center. It was noted that other rows of stacked lumber bundles, immediately adjacent to the stack that fell, were not stacked securely and were leaning outward. For reasons unknown, Employee #1 departed the forklift and walked over to the lumber stacks, where two bundles of stacked lumber fell forward and landed on top of him, crushing him to death. There were no witnesses to the accident.
