Employee's Leg Is Fractured Between Pallet and Equipment

At approximately 7:15 p.m. on April 26, 2006, Employee #1 was unloading packages from a 24 ft truck onto an Extendo Rapistian Demag conveyor belt, UNL-16, at the primary unloading area, Dock Number 13. He pushed the button to extend the Extendo conveyor belt, which kept extending and caught his leg between the end of the Extendo and a palette. He attempted to use the emergency stop button, which did not work. He broke his tibia and fibia in his left leg and was hospitalized.
