Investigation Summary
At approximately 9:00 a.m. on June 16, 2005, Employee #1, a street repair specialist, was assigned to hoist a manhole out of the street with his boom truck. As Employee #1 was performing a safety walk-around inspection, his foot slipped into the trench encircling the manhole, and he fell. The trench was approximately 16 inches wide by 12 inches deep and was cut through the asphalt so the manhole could be extracted. Employee #1 was admitted to the hospital for treatment of a shattered his left hip.
