Investigation Summary
At 10:30 a.m., on April 4, 2005, Employee #1, who was employed by Am-Sod Inc., at their Escondido site, was working on a tractor, a Kubota MX5000F, in the equipment yard. The tractor had been left in gear with the wheels unchocked. Employee #1 was standing outside of the vehicle, and reached in to turn the key. When the engine started, the vehicle moved forward, striking the employee, and running him over, crushing his left arm and knee. He was hospitalized for his injuries.
