Employee Amputates Finger in Brake Press Accident

On November 11, 2004, an employee worked for an aircraft parts manufacturer, and operated a Tri-Acro 11.22-ton brake press (Model 17-A, ID # H-31, Serial #1289P). The press was not equipped with point of operation guarding. The employee was positioned at the operator's station, manufacturing small parts, while sitting in a rolling chair. The chair moved unexpectedly while the employee was activating the press. His right index finger was caught in the press, which resulted in the amputation of his right index finger at the first joint.
