Employee is killed after forklift crashes into load bar

On August 25, 2003, an employee assembled pallet loads for a grocery warehouse operation. While operating a Crown PC 3000 pallet rider truck, the employee crossed from one aisle to another by traveling underneath a storage rack that was approximately 6.6 ft in height and 8.7 ft in width. In the process of going underneath the storage rack, the employee was struck by a load bar (four-inch beam) that was approximately 4 ft 11 in. from the ground. The load bar was located approximately 12 ft 7 in. deep in the 17-ft deep storage rack. Visibility under the storage rack was poor, which concealed the load bar that extended across the space. The employee was found with his feet off the floor, with his head and neck bent backwards. He was killed, with his neck pinned against the load bar and his body resting against the truck.
