Forklift truck backing up strikes employee and kills him

On May 26, 2004, an employee was seated on a Cushman 3-wheel utility vehicle. A forklift truck with no brakes backed into the Cushman Scooter, and crushed the employee between an angle iron discharge table and the scooter.
