Employee killed in fall through floor opening

On December 10, 2003, Employee #1 was engaged in installing the floor plates of a catwalk. Because the side stringers were not secured together, the plates spread apart, and a portion of the floor plates, where Employee #1 was working, fell out. He died when he fell 27 feet 4 inches into a water puddle approximately 4 to 6 inches deep, striking his head on a 2 inch by 4 inch, 6 foot long, piece of lumber.
