Employees Are Burned in Phenol Pipe Failure

On July 11, 2005, Employees #1 through #3, with Plastic Engineering Company, were removing a blockage in a phenol pipe line. Prior to removing the pump, the employees had verified the valves were working correctly, the valves were open, each end of the pipe section was open to atmosphere. In addition, pressurized steam was introduced on one end of the pipe section and observed leaving from the other end. When Employees #1 and #2 (maintenance workers) separated the pump head from the centrifugal pump casing base, a steam/phenol mixture sprayed all three employees. Employees #1 through #3 were hospitalized for thermal/chemical burns to their faces, necks and legs. The two maintenance workers were next to the pump and were wearing safety boots, typical work clothing, neoprene chemical resistant jackets, face shields and safety glasses. Employee #3 (an electrician) was standing in doorway approximately 10 ft away and wearing safety boots, typical work clothing and safety glasses. Emergency showers, additional water sources, and phenol soap were available within 20 ft of the incident.
