Investigation Summary
At approximately 2:00 p.m. on October 3, 2003, Employee #1, working for Donnell Construction, was framing a house on a construction site. He was standing on a 2-foot by 6-foot board that was part of a framed wall on the second floor and was nailing a 2-inch by 4-inch brace into a ceiling joist with a nail gun. While he was leaning on it, the brace slipped out of place and he fell 18 feet and was killed. Fall protection was not used by the employee.
