Employee killed when scissors lift falls

At approximately 12:40 p.m. on July 1, 2003, Employee #1 was working under a suspended scissors lift table. The table had been hoisted to the fully extended position by an overhead chain hoist. The base of the table was resting on the floor. The chain slings were attached to tabs on each side of the table top. The welds on the tabs broke, the table collapsed, and Employee #1 was crushed and killed. The tabs were .25-in by 3-in by 5-in nominal. The tandem lift table was equipped with maintenance bars that were designed to prevent the table from collapsing; however, the bars were not in place.
