Employee dies after being caught between paper rolls

Employee #1 was threading a roll of paper into the back side of an extruder when the roll of paper behind him rolled forward, pinning him between the two rolls of paper. The employee received a severe head injury and was transported to the hospital where he later died. The investigation revealed that the employee failed to chock the roll of paper behind him while he was threading the roll into the extruder.
