Investigation Summary

Investigation Nr: 201352234
Event: 08/19/2003
Employee dies after being struck by vehicle

On August 19, 2003, Employee #1, a landscaping crewmember, and other coworkers were trimming shrubs, grass and trees within a median strip of a housing community. Plants within the median divided the road in two. They parked the truck in one of the lanes of the road and activated its flashers, then placed a traffic cone approximately 25 ft behind the truck. None of the crewmembers wore high visibility clothing. Once the crew finished their work, they started gathering their items to relocate to another area. The traffic cone was removed from the road, and placed in the truck. Employee #1 was directly behind the truck, drinking water from a container near the rear of the truck. An individual driving his vehicle on the same road approached area where the truck was located. This individual waved to a group of people and became distracted. His vehicle struck the parked truck crushing Employee #1 between it and the parked truck. Employee #1 sustained traumatic injuries including extensive fractures, contusions and lacerations, and died at the scene from these injuries.
