Employee Is Burned When Wall of Mulch Collapses

On March 3, 2005, Employee #1, who was no longer supervising the fire fighting of an approximately 30-ft tall mulch pile, went back to the mulch pile and took his glove off. He walked up to the wall of mulch to see if the mulch was hot in that area. While Employee #1 was standing next the mulch pile, the other employee, who was driving a front end loader, and who was moving the burning mulch, saw the wall of mulch start to collapse. He called out to Employee #1. Employee #1 turned away from the mulch pile but the wall of mulch collapsed on top of him. Employee #1 sustained multiple burns and was hospitalized.
