Investigation Summary
At approximately 10:30 a.m. on December 29, 2003, Employee #1 was lifted on the forks of a lift truck to assist in counting packages on shelving during the annual inventory process. Employee #1 lost his balance and fell approximately 8.5 feet from the elevated forks of the lift truck to the concrete floor of the warehouse. The employee died nine days later as a result of severe head injuries. The employer, Fox Valley Container, did not have a written standard operating procedure for this process.
