Employee falls from scaffold and dies

On July 15, 2003, an employee and a coworker were installing soffit on the cantilevered section of a new single family dwelling. They were working from a pump jack scaffold, with 24-in. high aluminum poles, and aluminum stages that were 16-in. wide, 16-ft and 20-ft long. They were working approximately 4.5 ft above ground level, however the employee was working on a 1-ft long stage over a basement area-way and stairway. Te distance to the lower level was approximately 11.5 ft. The 16-ft stage became displaced for unknown reasons, and the employee fell into the areaway, causing him to sustain severe head injuries. The employee died on July 21, 2003.
