Employee fell from a forklift platform and injured

On April 19, 2003, an employee of Pulioe Construction Inc. was riding in a forklift platform, trash box that was not secured to the forks or backrest of the forklift. The box tipped and fell off of the forks, throwing the employee out. The box then landed on top of the employee causing serious injuries including a punctured lung, broken ribs and head injuries. The employee was hospitalized.
