Event: 08/25/2004

Employee is killed in fall from scaffold

At 11:15 a.m. on August 25, 2004, Employee #1 fell from a scaffold. There were no witnesses. Employee #1 was measuring a stained glass window and getting a mold of the arch for the storm windows. Employee #1 was on a mobile scaffold with four five-foot sections with screw jacks and casters. No fall protection was available, and the employer did not ensure employees were knowledgeable or trained on scaffolding issues. The total height of the scaffold was approximately 22.5 foot tall. The top plank was approximately 21 feet from the ground level. The scaffold was not fully planked and had no guardrails. The second plank from the top was 17.5 feet from the ground. This level was not fully planked and had no guardrails. The third level had two planks, and again was not fully planked and had no guardrails. A pool of blood was noted, indicating that after falling from the scaffold Employee #1 landed on the platform and then rolled toward the building where he fell an additional 12.5 feet, the rest of the way to the ground. Employee #2 saw Employee #1 roll off the scaffold from the third level of planking and fall lifelessly to the ground. Employee #3 called Emergency Services. Employee #1 was pronounced dead at the scene by the Franklin County Medical Examiner.
