Investigation Summary

Investigation Nr: 200527638
Event: 02/15/2004
Employee Is Caught in Auger and Killed

On February 15, 2004, an employee and coworkers at a hog slaughter plant were cleaning the rendering area in preparation for the next day's production when the employee had both legs pulled into a 10-inch inclined auger that started unexpectedly. The point of entry into the auger was at an opening in the auger casing where additional chutes fed product into the auger. The employee was standing on the auger flights when the machine energized. The employee was killed. The auger was not locked out at the time of the accident.
