Employee is killed when forklift overturns

On July 30, 2003, Employee #1 was operating a Pettibone Rough Terrain Forklift, Model 636. Employee #1 lost control of the vehicle. The vehicle left the road and fell 20 ft down a ravine. Employee #1 was struck to the chest by the vehicle when it overturned on him. He was killed on the spot. The road was in the bad conditions with the soil washed away at the edge of the road. In addition, Employee #1 moved over to the side of the 20-ft-wide road to allow passage of a pick-up truck traveling in the opposite direction. Employee #1 was not wearing a seatbelt.
