Employee died when struck on head by tree limb

On May 17, 2003, Employee #1 was standing on a ladder using a chainsaw to cut an overhead limb (diameter of limb was 10 in.). The limb broke with approximately 2 in. left to cut. The limb struck the employee on the head, killing him.
