Employee killed in fall through ventilation shaft

On August 12, 2003, Employee #1 installing conduit in a mechanical room on the 10th floor of a building. He had apparently placed a piece of plywood on the floor, but it was not large enough to provide a stable surface. Employee #1 was using the top of a ventilation shaft as a work platform when the floor, which was made of drywall, gave way. He fell through the shaft to an internal damper near the second floor and was killed. Employee #1's body was found after the accident by a security guard.
