Investigation Summary
On December 30, 2003, Employee #1 was unloading product from a press. Employee #1 tried to retrieve a mis-fed piece of product and her hand was caught in the press with the die down. Employee #1 suffered a crushed left hand and was taken to the hospital where her hand was amputated later that evening. Employee #1 was hospitalized.
