Employee Is Crushed and Killed by Falling Jacking Cradle

On July 13, 2004, Employee #1, a maintenance worker, was raising a 3,600-pound jacking cradle on the feed end of the Semi-Autogenous Grinding (SAG) Mill. The cradle became out of balance and fell, crushing and killing the employee. Further investigation revealed that the cradle had not been stabilized with cables around the SAG mill or secured to the other cradle with channel iron. Both of these procedures are written manufacturers recommendations when raising or lowering the mill.
