Employee killed when run over by improperly parked van

On November 17, 2003, Employee #1 was operating a company vehicle, a commercial-type van. He was using it to pick up tools to be used on the job. He arrived at his residence and did not turn off the engine. He did not put the automatic transmission selector level completely into park. He exited the vehicle and fell as the vehicle started to move and he was run over and killed.
