Investigation Summary
On August 18, 2006, Employee #1 was working in the processing plant of Paramount Citrus. He was moving oranges by hand on the carousal bag machine, when his right index finger became caught in the pinch point between two conveyors of the machine. He suffered an amputation of the tip of his right index finger and was hospitalized. Further investigation revealed that the area that created the pinch point on the machine was not guarded.
