Investigation Summary
At approximately 2:00 a.m. on November 25, 2006, Employee #1 was a crew member of the janitorial staff for the Savemart grocery store. He had been lifted approximately 6.5 ft on a Big Joe pallet jack and was cleaning windows. Employee #1 was injured when he fell from the platform and struck his head on the concrete floor. He was hospitalized at St. Agnes Hospital.
