Employee's Foot Is Amputated by Flat Bed Trailer

On September 18, 2009, an employee, a truck driver, had delivered a water truck to a work site. He was reconnecting the lowboy trailer to his truck. The trailer was a WITZCO RG35 Drop Trailer Number 1WBA11E206S000053. This trailer was equipped with front-flip ramps to accommodate front end loading and unloading. The employee stated both in his written statement that he provided to the employer and during the interview conducted that he disconnected the trailer to unload a water truck using the front ramps and was reconnecting the trailer when the nose of the trailer fell on his foot. He could offer no explanation of what made the trailer fall. The employee was hospitalized for a foot amputation.
