Employee Is Injured During Conveyor Cleaning

On February 9, 2006, Employee #1 was cleaning the conveyor, which was neither locked out nor tagged out. While Employee #1 was placing a lid on the conveyor belt, a remote operator turned on the conveyor and the corner, of the lid held by Employee #1 became hooked on the conveyor belt. He was pushed violently against a concrete wall by the lid attached to the conveyor. Employee #1 sustained a serious injury to the groin area and had surgery. He was hospitalized.
