At approximately 1:00 p.m. on April 15, 2006, Employee #1 was working for an international food company, performing her regularly assigned duties. She tripped over a 1-inch thick cleaning hose and fell to a concrete floor, fracturing her right femur. Employee #1 was hospitalized and treated for her injury, which required surgery.
