Investigation Summary
At approximately 12:00 p.m. on August 18, 2006, Employee #1 and a group of coworkers were walking to the parking lot from the yard area of a lumber manufacturer. Employee #1 was further into the roadway than the other employees. A coworker, driving a Taylor forklift, Model Number TE200S, came behind the employees. As the employees approached a curved area, the forklift driver made a wide left turn to avoid them. He was looking to his left and did not see Employee #1 in front of him. The left arm of the forklift struck Employee #1 and knocked him to the ground, where he hit his head. He was transported by helicopter to Kern County Medical Center where he died. The driver of the forklift stated that he was in second gear and was traveling at approximately 8 to 10 mph.
