Investigation Summary
At 12:00 p.m. on March 27, 2006, Employee #1, with Corrwood Containers, was cleaning a tray former machine manufactured by Moen. She was removing glue from the sheet metal drop chute of the machine, while it was stopped. Employee #1 inadvertently activated the machine, causing a compound fracture of her radius and ulna. She was hospitalized at Keawh Delta Hospital for treatment.
