Investigation Summary
At approximately 10:45 on February 8, 2006, Employee #1 was operating the Roll Line Number 1- Cooling Station. He went around the backside of the equipment during an equipment failure. His arm was caught and injured by one of the rollers. Employee #1 was taken to University Medical Center, and his left wrist was amputated. He was hospitalized.
