Employee's Leg Is Amputated While Training to Use a Forklift

At approximately 6:45 p.m. on October 17, 2004, Employee #1, a recent hire, was undergoing the organization's initiation orientation and training. He had completed 3 hours of training on a stand up clamp type forklift. During a break period in the training, he and some other trainees of the class asked if they could drive a forklift to augment their operating skills and become more familiar with the machine. They were allowed to drive a forklift around in an open area of the building set aside for this purpose. While Employee #1 was operating the forklift it came into contact with one of the pillars in the training area. His left leg became caught between the pillar and the forklift, amputating it. Employee #1 was hospitalized.
