Investigation Summary

At approximately 3:30 a.m. on March 14, 2003, Employee #1, of Sun Studs Llc, had just clocked in at the lunchroom in the sawmill. He exited the sawmill and was on his way to the planer to start his work shift when he walked into the path of a forklift traveling in reverse with a full load of lumber. At the time of the accident, it was dark and raining with low visibility. The forklift driver did not see Employee #1 and ran over him with the rear wheels. Employee #1 was transported to a local hospital and then transferred to a hospital in Portland where he was treated over several months for a fractured pelvis, broken legs and other injuries.
