Investigation Summary
On November 12, 2002, Employee #1 was using a home-made ladder to retrieve supplies from a third-story-loft. On his way down, with the supplies, the ladder collapsed and Employee #1fell 20 feet to the ground. Employee #1 was hospitalized and pronounced dead due to injuries sustained.
