Employee died due to collapsing car incident

At approximately 10:00 a.m. on January 25, 2003, Employee #1, a parts puller at a salvage yard, was attempting to "pull" an alternator from a vehicle, Mazda Protege. The vehicle was raised up using a jack found in the car, which was resting on the frozen and ice covered ground. Employee #1 was lying underneath the vehicle, behind the front passenger tire, under the door section. The jack slipped on the ice and pinned Employee #1 between the vehicle and the ground. Employee #1 was hospitalized and pronounced dead due to injuries sustained.
