Investigation Summary

Investigation Nr: 200712404
Event: 01/19/2005
Employee Dies in Fall from Roof

On January 19, 2005, Employee #1 was positioning bundles of roofing material on the roof of a residence as other members of the crew were located elsewhere on the roof. The pitch of the roof was 12-in-12 (vertical to horizontal). Employee #1 fell approximately 23 feet (7 meters) from the roof onto a concrete driveway. Although a lifeline was tied to an anchor point at the ridge of the roof and a body belt (not a body harness) was attached at the other end, he was not wearing the body belt. Employee #1 died later in a hospital.
