Employee falls from ladder

On December 10, 2003, two employees were removing existing eight foot fluorescent light fixtures on the ceiling of an old store being renovated. The two employees were working from two 12 foot A-frame ladders. Employee #1 was cutting the chain attached to each fixture, as Employee #2 cuts the power and removed the fluorescent tubes. The fixtures were connected in groups of up to six units. When Employee #1 cut the second chain of a group of four units, the units fell, knocking him to the ground 8 feet below. Employee #1 was hospitalized with head injuries, and died the following morning.
