Employee killed in fall from tree

Employee #1 was preparing to top and remove a 40- to 50-foot diseased oak tree. He was in the tree approximately 25 feet high when he unhooked his safety strap and went around to the opposite side of the tree to make a final cut. He did not reattach his safety strap and when he made the final cut he was tied-off above the cut line. When the tree fell, he was pulled over with the tree top and landed head first onto a stack of logs and tree brush that was on the ground. He sustained severe head injuries and was transported by helicopter to a hospital where he died 7 days later.
