Investigation Summary
At approximately 7:57 a.m. on May 17, 2003, Employee #1 was correcting a fault with the B plow. After notifying the control room of the reset to the B plow, Employee #1 went to climb upon the equipment. A coworker heard a yell for help and witnessed Employee #1 go past him on the conveyor belt. Employee #1 hit the retractable shirt board with his head. His head, neck and body were pinned in an awkward position, making it impossible for Employee #1 to breath. He was pronounced dead at the hospital.
