Employee Suffers Amputation Using Litho Laminator

Employee #1 was the operator of a litho laminator and was changing over from a foam core board to a thinner E-flute paperboard. Employee #1 was making a stop gate adjustment with the feed rollers still running. The stop gate is adjusted so only one sheet is fed at a time. Employee #1 was having trouble getting the stop gates adjusted down with a stack of boards on the feeder so he had all but one of them taken off. Employee #1 had his left hand on the sheet to maintain his balance and reached up with his right hand to crank down one of the stop gates. The feed rollers caught the sheet and pulled the sheet, along with it Employee #1's left hand, into the machine. Employee #1's index finger and middle finger were caught in the ingoing nip point and crushed badly. Employee #1 suffered multiple fractures in both fingers and the index finger required vascular surgery. Employee #1 was hospitalized.
