Investigation Summary
On December 20, 2002, Employee #1 was attempting to make a mechanical adjustment to a shrink wrap machine when the machine was started, trapping him between the rollers and the wrapping mechanism. He was crushed at the point of operation by the machine. The employee died from his injuries.
