Employee injured when struck by overhead hoist equipment

On April 25, 2003, Employee #1 was struck in the lower back by the displacement of the overhead hoist equipment that was elevated approximately 17.5 feet in height. Repairs and adjustments were being performed on the hoist equipment that was not locked out and no warning signs were placed on the hoist equipment by a designated person. Employee #1 was struck by the overhead hoist equipment while working under the hoist monorail system. Employee #1 did survive and was sent to the local hospital and released with no body fractures or serious internal injuries.
